Getting NCMIC's Malpractice Insurance Plan in place isn't difficult or complicated. You can do it right now with our online application.

Please have the following information available to begin. Once you have started, you cannot save the application to finish later, but you can choose to provide required documents at a later time.

  • Licensure Information – Each active state license number, issue date and certification you hold.
  • Previous Insurance Information – A copy of your current Declarations Page (if you are currently insured).
  • Payment Information – Bank account or credit/debit card account information for required premium payment.

When coverage is effective:

Upon approval of your application, your policy effective date can be as early as the day after your completed application is received by NCMIC.

If we have questions when processing your coverage request, we'll let you know. If we have all of the information we need when you submit your application, we'll generally have it processed within 5-7 business days.

As a proof of coverage, we will automatically fax or email your Declarations Page once your policy is issued.

The NCMIC Malpractice Insurance Plan is underwritten by NCMIC Insurance Company. In the states of Florida and New York, the NCMIC Malpractice Insurance Plan is issued by NCMIC Risk Retention Group, Inc.
